\u672c\u4eba\u540c\u610f\u5b69\u5b50\u53c3\u52a0\u7531\u57fa\u7763\u4e4b\u5bb6\u7b2c\u4e94\u5bb6\uff08HOC5\uff09\u8209\u8fa6\u4e4b BLESS \u590f\u4ee4\u71df\u3002\u672c\u4eba\u7406\u89e3\u71df\u6703\u6d3b\u52d5\uff08\u5305\u62ec\u5de5\u7a0b\u5be6\u9a57\u8207\u9ad4\u80fd\u6d3b\u52d5\uff09\u5b58\u5728\u56fa\u6709\u98a8\u96aa\u3002\u672c\u4eba\u540c\u610f\u8c41\u514d HOC5\u3001\u5176\u6559\u8077\u54e1\u53ca\u7fa9\u5de5\u514d\u9664\u4efb\u4f55\u50b7\u5bb3\u6216\u640d\u5bb3\u8ce0\u511f\u4e4b\u6cd5\u5f8b\u8cac\u4efb\u3002\u82e5\u767c\u751f\u7dca\u6025\u60c5\u6cc1\u4e14\u7121\u6cd5\u806f\u7e6b\u5230\u672c\u4eba\uff0c\u672c\u4eba\u6388\u6b0a HOC5 \u70ba\u6211\u7684\u5b69\u5b50\u5c0b\u6c42\u5c08\u696d\u91ab\u7642\u6551\u6cbb\u3002<\/p>\n

Participant Waiver and Release Form<\/strong><\/h3>\n

1. Liability Waiver & Medical Consent<\/h4>\n

I give permission for my child to participate in the BLESS Summer Camp at The Home of Christ Church (HOC5). I understand that camp activities (including engineering projects and physical play) involve inherent risks. I agree to release HOC5, its staff, and volunteers from any liability for injury or damage. In the event of an emergency, I authorize HOC5 to seek professional medical treatment for my child if I cannot be reached.<\/p>\n

2. Photo & Media Release<\/h4>\n

I grant BLESS Summer Camp and HOC5 the right to take photographs and videos of my child during camp activities. I agree that these materials may be used for promotional purposes (including social media, website, and brochures) to share the blessings of the camp with the community. No last names or private information will be attached to photos without additional consent.<\/p>\n

3. Privacy Policy<\/h4>\n

We collect personal information (names, contact info, allergies) solely for the safety and administration of the BLESS Summer Camp. We will not sell or share your data with third parties, except as required for emergency medical care or by law.<\/p>\n

4. Lunch & Allergy Notice (Full-Day Only)<\/h4>\n

For Full-Day participants elect to include lunch, parents are responsible for listing all food allergies during registration. While we strive to provide a safe environment, HOC5 cannot guarantee a 100% nut-free or allergen-free facility.
